Preparing for your First Visit

Our goal is to make your visit as smooth and efficient as possible, ensuring you see Dr. Kale without delay. To assist with this, please complete the electronic forms that were sent to your patient portal, or you may download the necessary forms below. All forms must be completed before your appointment. You can fill out all forms directly on your device of choice. Once completed from the patient portal, we will receive notification, and that is when we begin to prepare your chart for your upcoming visit. You may also print them, sign any documents that require a signature, and bring them with you to your scheduled appointment. We ask that you arrive at least 30 minutes prior to your scheduled appointment to ensure all documents are uploaded and your visit note is prepared for Dr. Kale. We appreciate your cooperation and understanding of our process.

• Photo ID
• Insurance Card(s)
• Method of payment for your visit.
• Physician referral forms if required by insurance
• A list of current medications if not already entered on the patient form.
• Medical records from a prior physician that may be helpful to Dr. Kale.
